Saron Sophia/Creative Collaborative Waiver

Saron Sophia is a community group partnering with non-profits to offer nature-based wellness activities to the public. In that we are a collective of individuals offering opportunities we request the following form be complete for participation.

Liability/Hold Harmless/Photo Release Agreement

I hereby release, waive, discharge, and convenient not to sue Saron Sophia/Creative Collaborative or it’s representatives from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss or damage to self or personal property in association with group or partnership activities.

In signing this release, I acknowledge and represent that I have read the forgoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed.

I am 21 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.
